MBDTC Class Registration Form

1. Download the Class Registration Form to the Adobe Acrobat Reader
2. Print the Class Registration Form
3. Fill out the form
4. Bring these items to the Registration Table at any Monterey Bay Dog Training Club, Inc. class location.
   • Completed Class Registration Form
   • Rabies Vaccination Records
   • DHLPP Vaccination Records
   • Registration Fee

Once you attend a Monterey Bay Dog Training Club, Inc. class, we will mail you a pre-completed Express / Registration form to save you even more time when you register for the next session!
